Integrated Continuous Care is one of the responses of the National Health Service, involving various technical levels and health and social support professionals.
The National Network of Integrated Continuous Care includes continuous health care and social support.
The multidisciplinary teams of the RNCCI, sharing knowledge and integrating care delivery, are responsible for developing consistent and appropriate responses to people's needs, facilitating rehabilitation processes, promoting autonomy and functionality.
The relevance of mental health in the RNCCI and the impact that care integration can have on people's quality of life are today's priorities and challenges for the future.
